We are grateful for all the community partners who are willing to participate in strategically problem solving, identifying resources and assigning support for individuals who are experiencing challenges at this time of transition.

One need that VLFA has identified that affects all of our communities is the organization of resources for individuals who are transitioning from the realm of public school services and supports to community based supports.
Our vision for how to meet this need is to create what we are calling the “Providers Collaborative”. The objective would be to create a multi-provider staffing to strategically problem-solve, identify resources, and assign support for individuals who are experiencing challenges during this transition time.

Can I refer someone who is not in transition age?
Yes, you can. The Provider's Collaborative's first priority is individuals in the transition stage, but the Collaborative will accept individuals of any age.
